Item The influence of the demodex mite on the morphological picture of eyelid papilloma(Georgian Association of Business Press, 2024) Artemov, О.; Lytvynenko, М.; Chumachenko, I.; Bondarenko, A.; Dotsenko, N.; Ostapchuk, K.; Koshelnyk, O.; Gargin, V.Aim: The aim of work is description of new observations related to the participation of demodex in tumor morphogenesis with goal to study the example of eyelid papilloma, imagine the connection between the presence of the demodex mite and certain changes in the typical histomorphological picture of the neoplasm. Materials and methods: Histomorphological examination covers biopsy and operative material of eye pathology with diagnosis of neoplasm (eyelid papilloma and actinic keratosis (senile keratosis, senile keratoma, solar keratosis)) were selected, with special attention paid to the presence of horn cysts. Agile methodologies were employed to manage the research workflow effectively. Results: As a result of the conducted histomorphological examination, in 24 cases (48%), patterns pathognomonic for demodectic infection were found. Such patterns, which could indicate the presence of a demodex mite, were cystic formations, as well as fragments of a dead parasite. We noted their presence not only in the tumor tissue, but also in the adjacent hair follicles, sebaceous glands, and subepidermal stroma. It is no coincidence that the authors who previously noted the presence of pigmented elements in keratopapillomas or seborrheic keratomas could not explain their origin, resorting to putting forward such fantastic assumptions as the presence of "symbiosis of melanoblasts and epithelial cells, which ensures the transfer of pigment from the first to the second". Conclusions: Histomorphological study of serial sections allows us to see the successive stages of transformation of cavity intratissue defects created by the activity of the mite into horn cysts. Cavity defects are the main evidence of demodectic infestation. They occur in basal cell and squamous cell carcinomas, xanthomas, sebaceous adenomas, etc. tumors of the eyelids, but only in keratoacanthomas and senile keratomas they can turn into corneal cysts, which can be assumed to be due to the differentiating potential of these neoplasms.Item Psychological features of the rehabilitation of persons in post-COVID-19 condition(Georgian Association of Business Press, 2024) Bondarenko, A.; Malieieva, O.; Malieiev, D.; Lantukh, I.; Filonenko, O.; Baiazitov, D.; Gulbs, O.The global medical problem has become the COVID-19 pandemic since 2019, which represents one of the most difficult medical realities. One of the crucial medical hypotheses is the effect of SARS-Cov-2 infection on mental health that requires creation of effective psychological and psychiatric management of such patients. In connection with described above, we set the task of our research to develop and test a complex of psychological interventions in the system of psychosocial rehabilitation of patients with cognitive disorders against the background of the endured SARS-Cov-2 infection. Materials and methods: The material of the study was the data obtained after passing the GAD-7 test for level of anxiety detection. The persons were divided into two groups with patients who have a history of severe acute respiratory syndrome of coronavirus 2 (SARS-CoV-2) and non-suffered with coronavirus. At the end of the course of psychological rehabilitation, the GAD-7 test was repeated. Results: It was established that people who have endured COVID-19 have a significantly higher level of anxiety during examining the initial level of anxiety by performing the GAD-7 test. So, if most of the examined had a minimal level of anxiety (68%) in the control group, then after COVID-19 the majority of patients had a moderate level of anxiety (64%), and 6% even had a high level of anxiety. A month after the initial testing, the group of patients (after COVID-19) who had not undergone psychological rehabilitation was still predominantly in the zone of moderate level of anxiety (54%). Going through psychological rehabilitation had positive consequences and the majority of patients moved into the minimal level of anxiety zone (58%). Conclusions: Majority of persons who endured COVID-19 suffer moderate level of anxiety with GAD-7 test 10.12±0.43 (6.92±0.33 for non-suffered). Psychological rehabilitation could reduce it to 7.24±0.36. The data obtained in the course of the work testify to the effectiveness of conducting a complex of psychotherapeutic interventions using cognitive training, cognitive-behavioral psychotherapy in the system of psychosocial rehabilitation of patients with cognitive disorders against the background of COVID-19 with reducing level of anxiety. Results provide critical information on the efficacy of psychological rehabilitation for persons who experience persistent cognitive deficits after COVID-19.Item Psychological features of rehabilitation of HIV-infected patients(Georgian Association of Business Press, 2024) Lantukh, I.; Kucheriavchenko, V.; Yurko, K.; Bondarenko, A.; Merkulova, N.; Mohylenets, O.; Gradil, G.; Bondar, O.; Bodnia, I.; Burma, Ya.; Tsyko, O.; Tkachenko, V.The human immunodeficiency virus (HIV) is one of the major medical problems in the world. For almost forty years, it has had catastrophic effects on the body of infected people with variable pathogenesis of mortality. The purpose of the work is to analyse the importance of psychological assistance for the social adaptation of HIV-infected patients. Materials and methods: The empirical basis of the study was the analysis of 30 medical records of inpatients. The majority were patients of working age, namely 25 people (83.3%), while the average age of the patients was (28.5±5.5) years old. During the study, patients were divided into groups depending on the receipt of psychological help, psychoemotional disorders, and signs of social maladjustment. Results: The first group of patients consisted of 10 (33.3%) patients who refused psychological help. The second group, namely 20 (66.7%) patients, consisted of patients who received psychological help immediately after the diagnosis of HIV infection and had milder disorders. During the research, a set of methods was used: clinical and anamnestic method; to assess the presence of depression in patients, the Montgomery-Asberg Depression Rating Scale (MADRS) was used; the Spielberger-Khanin anxiety questionnaire was used to determine the level of anxiety in patients; patients' adherence to treatment was determined using the Morisky-Goin compliance assessment scale. Conclusions: The issue of adaptation of HIV-infected patients in society is a significant problem caused by the low level of awareness of HIV infection and the ways of its transmission. The obtained results indicate the need for psychological assistance to HIV-infected persons at every stage, both at the time of the announcement of the diagnosis (prevention of the development of depression, suicidal intentions), and during drug treatment (debriefing of the methodology, creation of peer-to-peer support groups), thanks to which the social adaptation of patients and the effectiveness of medical treatment are accelerated.
