    A Study of the Effect of Specific Antibacterial Therapy on the Course of Tuberculous Spondylitis in an Experiment

    Objective — to assess the impact of modern antibacterial therapy on the development of a specific destructive process in experimental tuberculous spondylitis in animals. Based on the histological examination of the vertebral bodies, it was established that the animals treated with specific first-line ABD showed clear morphological features of tuberculous inflammation. In animals treated with specific second-line ABD, inhibition of the pathological process. This study showed that the implementation of modern intensive specific antibacterial therapy in experimental conditions allows to achieve the delimitation of the destructive process in the relatively early stages of the disease development (4—5 weeks). The obtained new knowledge about the pathomorphological features of the course of TS against the background of carrying out specific antibacterial therapy allows radical surgical interventions on the musculoskeletal system without the risk of generalization of the tuberculosis process at an earlier time (compared with the generally accepted ones).

    The new method of TS treatment in the main group compared to traditional standards of surgical interventions in the control group allowed: 1) to avoid the progression of TS, the development of complications of the disease (p < 0.05) and complications related to the transplant (fractures, displacement of the autograft, cage) (p < 0.05), in the early postoperative period; 2) significantly increase the effectiveness of treatment due to an increase in excellent results (46.7 and 26.7 %, respectively) and a decrease in satisfactory (13.3 and 23.3 %) and unsatisfactory (0.0 and 10.0 %; p < 0.05) of the results in the remote terms of observation; 3) significantly shorten the inpatient stage of treatment ((96 ± 12) and (190 ± 21) bed-days; p < 0.001).

    The Results of Surgical Treatment of Patients with Active Tuberculous Spondylitis

    Проведено аналіз результатів оперативного лікування 60 пацієнтів з активним туберкульозним спондилітом. В основну групу (n=30) включені пацієнти з ураженням грудних та поперекових хребців, оперовані із застосуванням телескопічного титанового кейджу для вентрального міжтілового спондилодезу після проведення некрсеквестректомії. Особливістю передопераційної підготовки пацієнтів основної групи було обов'язкове проведення етіологічної діагностики перед початком лікування, призначення короткострокового передопераційного курсу специфічної антибактеріальної терапії з урахуванням результатів етіологічної діагностики протягом 2–3 тижнів з подальшим проведенням оперативного втручання. До контрольної групи увійшли пацієнти (n=30) із застосуванням традиційних підходів до лікування – передопераційна підготовка з використанням 3–5 специфічних антибактеріальних препаратів без визначення чутливості протягом 2–3 місяців з наступним оперативним втручанням – декомпресивна некректомія уражених хребців та переднім спондилодезом аутотрансплантатом. При порівняльному аналізі найближчих та віддалених результатів хірургічного лікування пацієнтів обох груп отримано результати, що достовірно підтверджують високу клінічну ефективність запропонованої методики лікування пацієнтів основної групи з використанням сучасних досягнень вертебрології.

    Improvement of the Effectiveness of Treatment Patients with Tuberculosis Spondylitis

    The protocols of clinical and radiological examination, analysis of treatment of 60 patients with active tuberculous spondylitis have become the clinical material of this study. The main group included patients (n = 30) with TS of the thoracic and lumbar vertebrae operated with the use of a sliding titanium cage for ventral intercostal spondylodesis. The comparison group includes patients (n = 30) using traditional approaches to the surgical treatment of the TS. The comparative study of the effectiveness of treatment of patients of the main and control group with TS showed that the use of the proposed technique in patients of the main group made it possible to significantly improve the results of treatment. Tuberculosis spondylitis (TS) in the structure of bone-joint tuberculosis in adults occupies a leading position and reaches 40 - 61.5% and it presents the great medical and social problem

    Biomechanical substantiation for external fixation of the pelvis using rods with different thread hands

    The results of medical rehabilitation ofpatients with consequences of unstable pelvicfractures (UPF) depend upon many factors,among which the strength of fixation of thefragments that makes possible early recovery ofthe staticodynamic function of the lower girdle isparticularly important,External fixation rod devices are widely usedin treating fresh UPF and their consequences,but it is not always that strength characteristicsof these devices make it possible to realize moderntendencies in medical rehabilitation, which call forearly verticalization of the patients and use ofconstant passive movement and application ofelectromechanical splints. This is explained by thefact that in patients with UPF, who have underwentexternal osteosynthesis with a rod device, bothaxial load and repeated cycles of hip flexion andextension reduce the strength of rod fixation inthe iliac bone.

    Mathematical Modeling of Changes in ‘Lumbosacral Region of the Spine–Pelvis’ Kinematic Chain in Iliolumbar Ligament Syndrome

    The pain in the lower back in several ocasions can be caused by iliolumbar ligament pathology. At a certain stage of pathogenesis of iliolumbar ligament syndrome recurring microtraumas of its fiber cause to their fibrosis that has an influence on stiffness of injured structure and biomechanics of kinematic chain ‘lumbosacral region of the spine–pelvis’. With a view to substantiating pathogenetic role of stiffness increase of iliolumbar ligament in pain formation in the lower back, by us there were made investigations of stress-strain state of kinematic chain ‘lumbosacral region of the spine–pelvis’ in iliolumbar ligament syndrome.
