    У сучасній системі охорони здоров’я, що характеризується зростанням вимог до професійної підготовки та діяльності медичних працівників, а саме лікарів, виникає потреба вивчення дисципліни «Основи біоетики та біобезпеки», що дасть майбутньому фахівцю оптимальний обсяг знань і дозволить грамотно приймати біоетичні значимі рішення при здійсненні професійної діяльності Вивчення дисципліни «Основи біоетики та біобезпеки», забезпечить становлення випускника як розвиненої особистості з активною життєвою позицією, розвиненою біоетичною свідомістю і високою біоетичною культурою; сформує у випускника професійні навики, що дозволяють вирішувати біоетичні значущі професійні завдання відповідно до видів професійної діяльності. Майбутній спеціаліст оволодіє здатністю і готовністю здійснювати свою діяльність з урахуванням прийнятих в суспільстві моральних і правових норм, дотримуватиметься правил лікарської етики, законів і нормативно-правових актів по роботі з конфіденційною інформацією, зберігатиме лікарську таємницю. Набуде здатність і готовністю реалізувати етичні та деонтологічні аспекти лікувальної діяльності в спілкуванні з колегами, середнім і молодшим медичним персоналом, дорослим населенням і підлітками, їх батьками та родичами. В результаті вивчення дисципліни «Основи біоетики та біобезпеки» студент знатиме - закони, принципи, правила і моделі біоетики; морально-етичні норми, правила і принципи професійної лікарської поведінки, етичні основи сучасного медичного законодавства; - основні етичні документи міжнародних організацій, вітчизняних і міжнародних професійних медичних асоціацій; принципи ведення дискусій в умовах плюралізму думок і основні способи вирішення конфліктів. Вмітиме вибудовувати і підтримувати, поважні і довірчі відносини з пацієнтами робочі відносини з членами колективу, а також конструктивні відносини з інститутами громадянського суспільства; аналізуватиме свою діяльність в аспекті захисту майбутніх поколінь, довколишнього середовища, біосфери і біорізноманіття; аналізувати біоетичні проблеми інноваційних технологій, реалізації репродуктивних прав людини, інфекційних захворювань, трансплантації, вмирання і смерті. Володітиме навичками викладання персональної точки зору, морально-етичної аргументації, ведення дискусій і круглих столів. Підсумовуючи викладення, хочеться наголосити, що у динамічному сьогоденні проблема біоетики та біобезпеки багатогранно відображена в різних сферах системі охорони здоров’я і вичення дисципліни «Основи біоетики та біобезпеки» є виправданим та необхідним.

    Waist to height ratio in hypertensive patients

    Overweight and obesity are common pathological conditions in the adult population of Ukraine and are associated with other chronic diseases, including hypertension, diabetes and dyslipidemia. All these diseases are key factors modified risk for other diseases, such as cardiovascular and renal disease. Early detection of these conditions will help in controlling their strategy. The aim of our study was to investigate the association of waist to height ratio (WHtR) in conjunction with the parameters of lipid metabolism in hypertensive patients. Methods. 73 hypertensive patients were examined. All patients underwent complex clinical examination which included estimation of complaints, history taking, physical and additional methods of examination. Following anthropometric parameters (WHtR) and parameters of lipid metabolism were measured. Data is represented as Me (Q25-Q75). Kruskal-Wallis and median test were used, р <0,05. Results. The hypertensive patients are characterized by increased levels of total cholesterol 5,67 (4,96- 7,11) mmol/L, triglyceride 2,75 (2,56-3,01) mmol/L, low-density lipoprotein-cholesterol 0,98 (0,83-1,35) mmol/L and decreased levels of highdensity lipoprotein-cholesterol 3,51 (2,77-4,54) mmol/L. Obesity was observed in 86% of hypertensive patients as measured by WHtR ≥0,5, dyslipidemia was detected in 73% patients as measured by levels of total cholesterol. In correlation analysis positive relations between WHtR and levels of total cholesterol (R=0,42; р<0,05) in hypertensive patients were found. Conclusions. The hypertension is accompanied by dyslipidemia, characterized by increased levels of total cholesterol, triglyceride, low-density lipoprotein-cholesterol and decreased levels of highdensity lipoprotein-cholesterol. Significant positive relationships between WHtR and levels of total cholesterol in hypertensive patients were detected, which is another confirmation of the clinical significance of this measurement and substantiates the need for screening patients with WHtR in determining obesity, dyslipidemia and cardiovascular risk

    Diastolic Dysfunction: from Discovery to the Last Updates

    Over a long period, chronic heart failure was primarily associated with impaired contractility of the myocardium of the left ventricular, which is a manifestation of systolic dysfunction. Based on modern ideas about the pathophysiology of CHF syndrome, systolic dysfunction is considered as one of the etiological factors along with changes in wall tension and the structure of diastolic filling, i.e. everything that is included in the concept of "LV remodeling". The objective is to present a review of the key parameters of chronic heart failure and left ventricular diastolic dysfunction according to the last updates. Material and methods. The role and significance of diastolic dysfunction as the cause of chronic heart failure syndrome, have only been started to be determined over the past three decades, which can be explained by the difficulty of early diagnostics of diastolic dysfunction. The article describes the historical evolution of estimating heart failure and left ventricular diastolic dysfunction. It is important to note the high prevalence of diastolic dysfunction. The signs of myocardial diastolic dysfunction are detected in people with almost any heart disease. The universal review of the most important parameters which determine this disorder has been presented. It has been shown that nowadays Doppler echocardiography is the noninvasive “golden standard” for estimation of diastolic dysfunction. We have presented methods of treatment for diastolic dysfunction, based on updated recommendations of the American College of Cardiology / American Heart Association in treating patients with heart failure in 2017. Conclusions. The experimental studies show that the survival rate of patients with heart failure with a preserved ejection fraction depends not so much on its indices, but on the severity of diastolic dysfunction. Diastolic dysfunction in patients with heart failure is a more prognostic mortality rate than the ejection fraction, which needs further research for better treatment.
